What is a DWI/DUI Evaluation?

A DWI/DUI evaluation is a clinical assessment that examines an individual's alcohol and/or substance use history and other factors relevant to the referral.

The evaluation is designed to help identify whether there are concerns related to substance use and whether additional education, treatment, monitoring, or other recommendations may be appropriate.

Depending on the circumstances, the evaluation may include:

A detailed conversation about your history, current circumstances, and concerns.

Substance Use History

Discussion of alcohol and/or other substance use, patterns, consequences, and related experiences.

Standardized Screening

Use of appropriate clinical screening or assessment tools.

The purpose of an independent clinical evaluation is to assess your individual circumstances and provide recommendations based on the information gathered.

Clinical findings and recommendations cannot be predetermined.

The evaluation fee includes:

Assessment

  • The fee for an OASAS-approved Impaired Driver Clinical Evaluation is $375. Payment is due in full prior to the scheduled evaluation. The evaluation fee includes the clinical assessment, review of required documentation, and completion of

    applicable reporting requirements. Additional services, including laboratory or toxicology testing when required, may involve separate fees.

    Payment for an evaluation does not guarantee a specific diagnosis, recommendation, or outcome.

  • At least 48 hours' notice is required to cancel or reschedule an evaluation. Cancellations made with less than 48 hours' notice and missed appointments are subject to a $100 fee.

    Clients who provide at least 48 hours' notice may reschedule their appointment without penalty. Additional details regarding cancellations, rescheduling, and refunds will be provided at the me of scheduling.

    Addi onal services, if clinically indicated, are separate and will be discussed beforehand.

What about confidentiality?

Evaluations are conducted with respect for applicable privacy and confidentiality requirements.

Because a DWI/DUI evaluation is a clinical evaluation rather than ongoing psychotherapy, the purpose of the service and the

applicable limits of confidentiality may differ from those associated with traditional therapy.

Before the evaluation begins, you will receive information about the nature of the evaluation and applicable confidentiality considerations.

  • Most evaluations take approximately 60–90 minutes. Additional me may be required for review of documentation, collateral information, and completion of applicable reporting requirements.

  • The evaluation fee is $375. Payment is due in full prior to the scheduled appointment. Additional costs, such as toxicology or

    laboratory testing when required, are not included in the evaluation fee.

  • No. You do not need an attorney to schedule an evaluation. Individuals may contact Graceful Therapy, LCSW, PLLC directly to inquire about scheduling and required documentation.

  • With your written authorization, information regarding your evaluation may be released to your attorney or another authorized party as appropriate. Applicable reporting requirements will also be completed in accordance with OASAS requirements.

  • At this me, impaired driver evaluations are conducted in person. Informa on regarding the appointment location will be provided when scheduling.

  • No. A diagnosis or clinical conclusion is never predetermined. Each evaluation is individualized and considers the information obtained during the assessment, relevant documentation, and your individual history and circumstances.

  • No. Recommendations are based on the information gathered and reviewed as part of the evaluation process and cannot be determined in advance.

  • Graceful Therapy, LCSW, PLLC currently provides impaired driver evaluations and does not provide substance use disorder treatment services. If treatment is recommended following an evaluation, appropriate referral options may be discussed.

  • Impaired driver evalua ons and ongoing psychotherapy are separate professional services. To maintain appropriate professional boundaries and objectivity, completion of an evaluation does not establish an ongoing therapy relationship with the evaluator. When ongoing treatment or additional services are indicated, appropriate referral options may be provided.
